Chris Morris, MD is the CEO and owner of CRM Physicians. He is board certified in Internal Medicine. He earned his medical degree from American University of the Caribbean. Completed residency at Maricopa Medical Center/Phoenix Childrens Center in Phoenix, AZ
Dr. Morris has many roles in the organization taking an active interest in administration, credentialing, medical billing and provider relations. He continues to round on patients at least 3 days a week. 
Dr. Morris has two boys and enjoys spending time with family, traveling, skiing and exercising. 

David P. Larson, MD, COO
David P Larson, MD is the COO and owner of CRM Physicians. He is a board certified Internal Medicine physician. He received his medical degree from Sidney Kimmel Medical College and completed his residency at Banner - University Medical Center. Dr. Larson has an active interest in Provider Relations and Operations Management. He rounds on patients at least three days per week. 
David and his wife, Kara, have 3 daughters. He enjoys exercising, cooking, brewing and spending time with his family.

 Adair Moore is a Nurse Practitioner and the Director of Business Development for CRM Physicians. Adair graduated with her MSN from Vanderbilt University in 2009. Adair started working in skilled nursing facilities in 2011 with Wellness Solutions in Nashville, TN. While working for Wellness Solutions, Inc, Adair was promoted to Eastern Division Services Director and was responsible for acquiring new contracts at skilled nursing facilities in East TN and Northern GA, interviewing and training new employees and writing a medical manual for NPs working in skilled nursing facilities. Adair moved to Portland, OR in 2013 to work with Optum Care Plus before joining CRM Physicians in 2014. Adair was promoted to Director of Business Development in 2016 and is now responsible for establishing contracts with new facilities, hiring and training new providers, coordinating with insurance companies,and managing our electronic health record and MACRA objectives. In Adair’s free time, she enjoys spending time with her husband and son, traveling, running, hiking and exploring the Pacific Northwest. 

Wendy Gallagher, NP, 
Employee Wellness Coordinator,
Research Analyst 
Wendy earned a Doctor of Nursing Practice (DNP) degree with a specialty as a Family Nurse Practitioner at University of Portland in 2016. Her undergraduate education was also at University of Portland where she earned a Bachelor’s degree in Biology in 2007 and another in Nursing in 2010. Prior to earning her NP degree she worked as an RN in a skilled nursing facility.  
In 2017 Wendy was promoted to Employee Wellness Coordinator helping our healthcare providers explore work-life balance. Wendy is putting her Doctorate to use by researching hospital readmission trends. She enjoys spending her free time with friends and family. Favorite hobbies include attending local festivals and music events.

 Caitlin Iacobacci, FNP-C
Caitlin Iacobacci, FNP-C, is a Family Nurse Practitioner. Caitlin graduated with her MSN from Wilmington University in 2017. Preceding her Master’s degree, she achieved her BSN from Neumann University in 2011. Prior to relocating to the Portland area, she worked as an emergency room nurse at a level II trauma center in Pennsylvania for six years. During that time, she became a board certified emergency nurse (CEN). Her experience in the emergency room setting peaked her interest in the care of the patient across the lifespan. Caitlin’s dedication to her patients, their disease processes, health, and well-being lead her to the path of becoming a Family Nurse Practitioner. Caitlin presently loves living near Portland and in her spare time enjoys exploring the Pacific Northwest, hiking, and traveling.

Christina Bazan-Grow, FNP-BC
Christina Bazan-Grow, FNP-BC, is a Family Nurse Practitioner. Christina received a BSN and a MSN (Advanced Practice Nursing) from the University of Nevada, Las Vegas. In addition, Christina earned a Doctor of Nursing Practice degree from Capella University, Minneapolis, Minnesota. 
Christina spent several years as a hospitalist in both acute care and psychiatric care and now she is primarily involved with skilled sub-acute and long term patient care. Christina loves working with memory impaired patients and has written several articles and two books regarding the care of those that have memory impairment. 
Christina donates her time to Doctor's Without Borders annually. In her free time she enjoys painting, white water kayaking, paddle boarding and hiking. 

Crystal Gilbert, PA-C
​Crystal Gilbert obtained her Masters Degree in Physician Assistant studies at Midwestern University in Glendale, Arizona. Prior to relocating to the Portland area in 2015, Crystal worked and lived in Phoenix, Arizona. Her husband's job transfer brought them to the Pacific Northwest. When not working, Crystal and her husband spend their time chasing a toddler around. Other ​interests include exploring Oregon, hiking, sewing and making jewelry.

Emily Forsyth, PA-C
​Emily Forsyth is a certified Physician Assistant and a Registered Nurse. She obtained a B.A. degree in Physiology from University of California, Berkeley, CA, and a Bachelor of Science degree in Nursing from Samuel Merritt College in Oakland, CA. Emily gained acute care experience as a registered nurse at Portland Providence Hospital in Portland and Southwest Hospital, Vancouver, WA, and in community-based nursing at Clark County Health Department Clinic and Infectious Disease departments. She returned to school to earn her Master of Science degree in Physician Assistant Studies from Pacific University in Hillsboro, OR.  
Prior to joining CRM Physicians, Emily was employed by Pacific Gastroenterology, and at Columbia Asthma & Allergy, both in Vancouver, WA. Favorite activities include dancing, hiking, and international travel. 

Erik Sasovetz, DO
Dr. Sasovetz is originally from the Wichita Kansas, graduating from the Kirksville College of Osteopathic Medicine in 2007. Following medical school, he completed an Internal Medicine internship at Portsmouth Naval Hospital before being assigned to Camp Lejeune, NC as a general medical officer with Marine infantry units. He completed two overseas deployments before leaving North Caroline, then moving to Portland Oregon to complete a Family Practice residence with FMSW in Vancouver Washington. His most recent employment prior to starting at CRM was as a general practitioner with The Vancouver Clinic and a Medical director at two SNF’s in Vancouver Washington. He is accompanied by his lovely wife Melissa and their two rescue dogs Gatsby and Lexi. 

Odessa Steigleder, MS, PA-C
Odessa Steigleder, MS, PA-C is a Physician Assistant certified by the National Commission on Certification of Physician Assistants. She obtained her Bachelor of Science degree from Oregon State University, Corvallis, Oregon and her Master of Science degree from the School of Physician Assistant Studies at Pacific University, Hillsboro Oregon.

Odessa received clinical education in Neurosurgery at Hillsboro Neurosurgical Associates, Hillsboro, Oregon, Emergency Medicine at Community Hospital of Grand Junction, Colorado, Internal Medicine at the University of Texas Medical Branch, Galveston, Texas and at Tacoma General Hospital, Tacoma, Washington. In addition she received training in Cardiology at Cascade Cardiology, Salem, Oregon, Family Medicine at Oak Harbor Naval Hospital, Oak Harbor Washington, Primary care Medicine at Indian Health Services of Browning, Montana and Community Medicine at Clinica Esperanza of Roatan Honduras.

Prior to joining CRM Physicians, LLC, Odessa was employed by the Cascade Cardiology in Salem Oregon.

Reginald Buesa, MD 
Reginald Buesa graduated from the University of Hawaii School of Medicine and completed his medical training at the University of Hawaii Integrated Medical Residency Program. He is board certified by the American Board of Internal Medicine. Reginald has practiced medicine for over 35 years and has taken care of patients in an outpatient clinic, hospital, nursing home and care home setting. In his free time, Reginald enjoys spending time with his family, reading, yard work and watching major sporting events. 

Renee Reed, NP-C
Renee Reed is a dual board certified adult and geriatric nurse practioner. She graduated with honors with her Baccalaureate in Nursing from OHSU at Southern Oregon University in 1996. She worked as a critical care nurse for nearly 19 years with 11 of those years at OHSU in Portland. Renee further earned her Master's in Nursing from University of Phoenix in 2005 and her Post Master's Certificate from UMASS Boston's dual track adult geriatric nurse practitioner program in 2011 with high honors.  Renee also earned her legal nurse consultant certification at Kaplan University and did legal nurse consulting work for several medical malpractice attorneys. Renee lives in Portland with her husband, Brent, a firefighter/paramedic for Tualatin Valley Fire and Rescue. Renee enjoys traveling, running, writing, family, friends, and volunteer work in her spare time. 

Robyn Trasen, PA-C
Robyn is a Physician Assistant who grew up in Denver, Colorado, and earned a Bachelor of Science degree from Colorado State University in Fort Collins, Colorado. She attended graduate school at Pacific University in Hillsboro, Oregon, and earned a Master of Science in Physician Assistant Studies. 
Robyn Trasen joined CRM Physicians in 2014. Her responsibilities focus on internal medicine, geriatric, long-term and skilled nursing care at Avamere Rehabilitation of Hillsboro. 
In addition to her role with CRM physicians, Robyn assists as a Skills Facilitator for the School of Physician Studies at Pacific University.
Robyn is a member of the Oregon Medical Board, the American Academy of Physician Assistants, the American Geriatric Society.
In her free time, Robyn enjoys cheering from the sidelines of the various sports and activities that her children participate in, and hiking, skiing, and traveling with her husband.  

Steve Kaplan, MD
Steve Kaplan, MD earned his medical degree from Upstate Medical Center in Syracuse. He completed his residency at the County Hospital in Phoenix (Maricopa Medical Center) and is board certified in Internal Medicine.  While he was working as an attending at Maricopa Medical Center he met Chris Morris, who was completing his residency at the time, and they have been good friends ever since. Steve is married to his wife, Nancy, and they have one grown daughter. His favorite past times include travel, hiking, photography, listening to blues and jazz music, cooking and baking. 

Swapna Chenna, MD
Swapna Chennareddyga did her Internal Medicine Residency from the University Of Texas Medical Branch. She has been in practice since 2009 as a primary care physician and a hospital based internist. Since 2013, she has followed her passion and has focused on internal medicine in skilled nursing facilities. In her free time, Swapna loves gardening, hiking and spending time with her two daughters.

Wendy Kellam, PA-C
Wendy Kellam moved to Portland, OR from Kalamazoo, MI in 2008. She received her degree in Physician Assistant Studies from Pacific University in 2013 where she proceeded to work in a large gastroenterology practice at The Oregon Clinic and Providence St. Vincent. Her prior experience in the SNF setting as a Speech Language Pathologist made CRM Physicians a natural fit. In her free time, Wendy loves exploring the Portland food scene with her partner Tom. She is a supporter of animal rescue and has a retired greyhound named Stanley.
