We are a team of skilled healthcare professionals united to meet the challenge of delivering compassionate evidence based medical services to patients residing in or transitioning through skilled nursing and intermediate facilities. We strive to provide unmatched medical care, specially tailored to the needs of each individual patient and their families.

What can I expect from CRM Physicians healthcare providers while under their care in a Skilled Nursing Facility?

Depending on the facility you are in, you can expect to be visited by one of our healthcare providers within a week of admission, with follow-up visits as medically appropriate.

Our healthcare providers start by performing a thorough review of your health history and hospitalization records, if applicable, promoting continuity of care. 

Our team is available 24 hours a day, 7 days a week to manage your care. We prescribe medications, order appropriate labs and radiologic studies and address acute medical care needs.

We strive to work in conjunction with your Primary Care Physician (PCP) and/or sub-specialist, Physical and Occupational Therapists, Social Workers and Resident Case Managers. On discharge from the Skilled Nursing Facility, we provide communication with your PCP, detailing your skilled nursing stay.


What is the benefit of having CRM Physicians care for me while in a Skilled Nursing Facility?

Our care prevents unnecessary and stressful rehospitalizations and decreases the number of days in the skilled facility

24 hour/day available medical care. The facilities are familiar with our team and so we are easy to reach in a crisis.

Our team is familiar with the nursing facility environment and regulations.

We visit you at the bedside, avoiding the need to transport to your Primary Care Physician

If I have a Primary Care Physician, do I have to be followed by CRM Physicians?

No. However, many Primary Care Physicians appreciate the help managing your care while you are in the Skilled Nursing Facility.

How Will CRM Physicians communicate with my Primary Care Physician?

Generally, communications are by phone or fax. We work hard to ensure pertinent medical records are sent to your physician on discharge from the Skilled Nursing Facility

What if I do not have a Primary Care Physician (PCP)?

The skilled facility where you are staying will help arrange one for you. Ideally, a PCP follow-up visit should be arranged before discharge from the Skilled Nursing Facility.

Will my insurance cover CRM Physician provider visits?

Yes, our business office and medical biller work closely with all major insurance companies.

Will CRM Physicians send me a bill?

It depends on your insurance. Some insurance plans require that we collect a small co-pay for each visit.

Does CRM Physicians have an outpatient practice?

Not at this time.

How do I get copies of my medical records?

Request them directly from the nursing facility.
